Wisconsin Premier Camp Registration

All participants are requested to sign the below release. Parent or Guardians must sign for minors. I/We, the undersigned, do hereby agree to allow the above named to participate in the a WPVBC, LLC volleyball Program/Camp (such as, Summer Sand, Bump & Jump, HS VB Indoor Camp, Tryouts, Tune ups, Club VB, Youth Academy and others). I am/We are aware of and understand that there may be potential risks inherent with participating in any recreational activities and that the WPVBC, LLC organizers (Fred Pramenko, Laura Leszczynski and Alex Uhan) and the Wisconsin Premier practice facilities and camp locations such as Galioto's Twelve21 do not provide accident insurance. I/We assume all risks and hazards incidental to such participation including transportation to and from the activities and do hereby waive, release, absolve, indemnify and agree to hold harmless the WPVBC, LLC, its affiliates, organizers (said Pramenko, Leszczynski and Uhan), volunteer helpers, staff and other persons for any all claims, injuries, liabilities, damage or right of action directly or indirectly arising out of the use of facilities or this organized play. In the event of a medical emergency, I authorize the organization staff to obtain medical treatment for the above named.

CAMP REFUND POLICY

In the event you wish you cancel more than three (3) weeks (21 days) prior to camp start date, a written request must be submitted to Wisconsin Premier either via email: [email protected] or mailed to: 1317 W Towne Square Road, Mequon, WI 53092 for a full refund less a $35 cancellation fee. Or you may receive a full credit to a future Wisconsin Premier event that may be transferred to family member or friend under the same terms and conditions as the original registration. If less than three (3) weeks prior to start date, no refund or credit will be issued. However, the amount may be transferred to a family member or friend under the same terms and conditions as the original registration. If the event is cancelled by Wisconsin Premier, you will receive a full refund or credit of any registration fees paid.
